Background:

  • Cellular blue nevi are typically benign pigmented skin lesions.

Observation:

  • A 14-year-old female presented with a pigmented cutaneous lesion on her leg, histologically consistent with a cellular blue nevus.
  • Synchronous massive inguinal lymph node metastases were identified.
  • Immunohistochemical analysis (S-100, HMB-45) and karyometric studies were performed on both the primary lesion and metastases.

Findings:

  • The primary cutaneous lesion exhibited cytologic and architectural features of a cellular blue nevus.
