Summary

Docetaxel, an antineoplastic agent, commonly causes sensory neuropathy. Higher cumulative doses increase the risk of severe neurotoxicity, indicating a dose-dependent effect.

  • To evaluate the correlation between cumulative docetaxel dose and neurotoxicity.
  • To determine the reliability of Vibration Perception Threshold (VPT) in monitoring neuropathy.

  • Prospective assessment of neurotoxicity in 41 patients across four Phase II trials.
  • Evaluation using a clinical sum-score for symptoms and signs.
  • Measurement of Vibration Perception Threshold (VPT).
  • Grading of neuropathy severity using National Cancer Institute's Common Toxicity Criteria.

  • Docetaxel induced predominantly sensory neuropathy in 20 of 41 patients.
  • Mild neuropathic symptoms were common.
  • At cumulative doses >600 mg/m², 3/15 patients had moderate and 1/15 had severe neuropathy.
  • A significant correlation was found between cumulative docetaxel dose and post-treatment clinical sum-score (p=0.002).
  • No correlation was observed between VPT and clinical sum-score or cumulative dose.

Conclusions:

  • Docetaxel causes mild, predominantly sensory neuropathy in a high proportion of patients.
  • Neurotoxicity is dose-dependent and can be severe at higher doses.
  • VPT is not a reliable method for monitoring docetaxel-induced neuropathy.
