Summary

Primitive anorectal malignant melanoma is rare and often presents with symptoms mimicking hemorrhoids. Early diagnosis and surgical resection offer the best, though still limited, survival rates.

Background:

  • Primitive anorectal malignant melanoma is a rare diagnosis.

Observation:

  • Four patient cases of primitive anorectal malignant melanoma are presented.
  • Clinical presentation included pain, proctorrhagia, tumors, tenesmus, hemorrhoids, and altered bowel habits.
  • One case mimicked thrombosed hemorrhoids, highlighting diagnostic challenges.

Findings:

  • The prognosis for this condition is generally poor, irrespective of treatment.
  • Early diagnosis and abdominal-perineal resection offer the best chance for long-term survival (over 5 years).
  • Radiotherapy and chemotherapy may reduce lesion size but do not significantly alter the overall prognosis.

Implications:

  • Biopsy of all removed anorectal tissue is crucial for accurate diagnosis.
  • Increased awareness of this rare melanoma is needed among clinicians.
  • Further research into improved diagnostic and therapeutic strategies is warranted.
