Mitral valve procedures in ischemic regurgitation

Summary

This study on ischemic mitral regurgitation (IMR) found that mitral valve repair is often successful, but isolated ring annuloplasty has a poor prognosis in patients with severe left ventricular dysfunction. Prompt mitral valve replacement may be better for critically ill patients.

Area of Science:

  • Cardiovascular Surgery
  • Interventional Cardiology
  • Cardiac Echocardiography

Background:

  • Ischemic mitral regurgitation (IMR) presents a complex challenge in patients with coronary artery disease.
  • Understanding the mechanisms and surgical outcomes for IMR is crucial for patient management.
  • A restrictive definition of IMR aids in selecting a more homogeneous patient cohort for study.

Purpose of the Study:

  • To evaluate the surgical outcomes of mitral valve repair and replacement in patients with ischemic mitral regurgitation.
  • To analyze the mechanisms of IMR, including annulus dilatation and leaflet motion abnormalities.
  • To assess the impact of surgical techniques on postoperative results and long-term survival.

Main Methods:

  • Retrospective analysis of 41 patients undergoing mitral valve surgery for IMR between 1993 and 1995.

Related Experiment Videos

  • Utilized transesophageal echocardiography (TEE) for assessing mitral regurgitation severity and mechanisms.
  • Included intraoperative TEE with loading and afterload tests for diagnostic clarification in select cases.
  • Surgical interventions included mitral valve repair (Mvrep) and mitral valve replacement (MVR), with detailed analysis of repair techniques.
  • Main Results:

    • Mitral valve repair was performed in 90% of patients, with ring annuloplasty being the most common technique (73%).
    • Postoperative residual mitral regurgitation was observed in 11.8% of cases, primarily after isolated ring annuloplasty.
    • Hospital mortality was 14.6%, with higher mortality noted in patients undergoing isolated ring annuloplasty (14.8%) and those with impaired LV function.
    • At six months, 80.5% of patients survived, with 88% in NYHA class I or II.

    Conclusions:

    • Mitral valve repair, particularly with techniques beyond simple ring annuloplasty, can achieve good outcomes in IMR.
    • Isolated ring annuloplasty in the presence of significant annulus dilatation and impaired LV function carries a mediocre prognosis.
    • In critically ill patients with precarious conditions, a rapid mitral valve replacement may be preferable to a complex repair.
