Summary

Sensorineural hearing loss (SNHL) can occur in children after radiation therapy (RT) for posterior fossa tumors. This late-onset hearing loss, appearing 18-36 months post-RT, warrants careful audiologic monitoring.

Background:

  • Sensorineural hearing loss (SNHL) in adults post-radiation therapy (RT) is dose-dependent.
  • Pediatric SNHL following RT without chemotherapy is rarely documented.

Observation:

  • This report details four pediatric cases of late-onset, asymmetric SNHL.
  • Children received surgical resection followed by 50-54 Gy RT for posterior fossa tumors, with no chemotherapy.
  • SNHL developed 18-36 months post-RT in children with no tumor recurrence.

Findings:

  • Most patients exhibited high-frequency SNHL in the poorer hearing ear and fluctuating hearing in the better ear.
  • No audiometric pattern was definitively identified.
  • Possible causes include injury to cochlear neurons and outer hair cells in the basal turn of the cochlea.
