Area of Science:

  • Infectious Diseases
  • Cardiology
  • Microbiology

Background:

  • Prosthetic valve dehiscence can be a complication of cardiac surgery.
  • Endocarditis is a potential cause of prosthetic valve dysfunction.

Observation:

  • A 73-year-old man presented with acute prosthetic aortic valve dehiscence.
  • Intraoperative findings included aortic annular disruption and a subannular abscess, suggesting endocarditis.

Findings:

  • Cultures revealed Bacillus licheniformis, an aerobic, spore-forming, gram-positive bacillus.
  • The patient recovered after aortic valve replacement and specific antibiotic treatment for Bacillus licheniformis.

Implications:

  • Bacillus licheniformis should be considered a potential pathogen in prosthetic valve endocarditis.
