Summary

A rare case of a pediatric renal mass initially misdiagnosed as xanthogranulomatous pyelonephritis (XGP) was ultimately identified as an intrapelvic Wilms tumor. This Wilms tumor exhibited continuous extension into the distal ureter, a presentation not previously documented in medical literature.
