Summary

Acute cauda equina compression is a rare complication following spinal stenosis decompression surgery. Prompt diagnosis with MRI and surgical re-intervention can successfully resolve symptoms.

Area of Science:

  • Neurosurgery
  • Spinal Surgery
  • Neurology

Background:

  • Spinal stenosis surgery aims to relieve nerve compression but carries risks.
  • Previous literature discusses paraplegia causes post-surgery.
  • The etiology of acute cauda equina compression remains incompletely understood.

Observation:

  • A 32-year-old male presented with spinal stenosis.
  • Post-decompression surgery, he developed acute cauda equina compression.
  • Hourglass constriction of the cauda equina leading to ischemia was suspected.

Findings:

  • Incomplete decompression was identified as the cause.
  • Magnetic Resonance Imaging (MRI) effectively diagnosed the compression.
  • The patient experienced complete symptom resolution after revision surgery.

Implications:

  • Highlights a previously unreported complication of spinal decompression surgery.
  • Emphasizes the importance of thorough decompression to prevent nerve ischemia.
  • Recommends immediate MRI and surgical intervention for suspected acute cauda equina compression.
