Summary

Laparoscopy Assisted Vaginal Hysterectomy (LAVH) is an effective procedure with a low complication rate. Further research is needed to fully understand its long-term effectiveness in gynecological surgery.

Area of Science:

  • Minimally Invasive Gynecological Surgery
  • Surgical Techniques and Outcomes

Background:

  • Laparoscopic hysterectomy, introduced in 1989, has seen increasing data availability.
  • Existing reviews face limitations due to retrospective data collection from specialized centers.

Purpose of the Study:

  • To review published series of Laparoscopy Assisted Vaginal Hysterectomy (LAVH) up to June 1995.
  • To address shortcomings in procedural evaluation strategies and propose a classification system.

Main Methods:

  • Comprehensive review of 4502 published LAVH cases up to June 1995.
  • Analysis of procedural data, including uterine artery ligation, conversion rates, and complication rates.
  • Discussion on the need for a classification system based on anatomical dissection.

Main Results:

  • Laparoscopic uterine artery ligation occurred in 76.1% of LAVH procedures.
  • The mean conversion rate was 2.5%, and the mean major complication rate was 3.24%.
  • Most frequent complications included urinary tract lesions (1.42%) and reintervention for hemorrhage (0.78%).

Conclusions:

  • Laparoscopy Assisted Vaginal Hysterectomy (LAVH) appears to be an efficacious surgical procedure.
  • Further data are urgently required to establish the long-term effectiveness of LAVH.
  • A classification system based on anatomical dissection is suggested as clinically relevant.
