Summary

Vesicostomy effectively preserves upper urinary tract function and morphology in children, particularly those with neuropathic bladder. This urinary diversion is recommended, with closure after definitive treatment, except for neuropathic cases needing augmentation cystoplasty.

Area of Science:

  • Pediatric Urology
  • Nephrology
  • Surgical Procedures

Background:

  • Vesicostomy is a urinary diversion technique used in pediatric cases.
  • Indications include neuropathic bladder and posterior urethral valves.
  • Evaluating its impact on upper urinary tract (UUT) is crucial.

Purpose of the Study:

  • To assess UUT morphology and function in children post-vesicostomy.
  • To compare outcomes based on the underlying cause of urinary issues.

Main Methods:

  • Retrospective analysis of 27 children (1 month–12 years) undergoing Blocksom vesicostomy.
  • Categorization into neuropathic bladder (n=20) and posterior urethral valves (n=3) groups.

Main Results:

Related Experiment Videos

  • In neuropathic bladder cases, 95% showed renal function improvement and 56% improved vesicoureteral reflux.
  • UUT morphology normalized pre-definitive surgery in all patients.
  • One posterior urethral valve patient maintained renal function at 2 years; prolapse was the main complication (14.8%).
  • Conclusions:

    • Vesicostomy is a suitable urinary diversion for these pediatric conditions.
    • Closure is advised post-definitive treatment, with exceptions for neuropathic bladder requiring augmentation cystoplasty to manage UUT pressures.
