    Summary

    Vascular calcifications are common in adult dialysis patients, particularly those on dialysis longer. However, new calcifications rarely develop in dialyzed patients or children, suggesting complex progression factors.

    Background:

    • Vascular calcifications are a known complication in patients with chronic kidney disease.
    • The prevalence and progression of vascular calcifications in dialysis patients require further investigation.

    Purpose of the Study:

    • To assess the prevalence of vascular calcifications in adult and pediatric dialysis patients.
    • To investigate the incidence of de novo vascular calcifications in adult dialysis patients.
    • To compare vascular calcifications in dialyzed and non-dialyzed uremic children.

    Main Methods:

    • Cross-sectional X-ray analysis of forefoot and pelvis in 101 adult dialysis patients.
    • Longitudinal prospective study of de novo vascular calcifications in 50 adult dialysis patients.
    • X-ray and autopsy evaluation of vascular and visceral calcifications in 138 uremic children.

    Main Results:

    • Vascular calcifications were observed in 20% of adult dialysis patients, with higher incidence in those on dialysis longer.
    • Only 1 out of 50 adult dialysis patients developed new vascular calcifications during the prospective study.
    • Vascular calcifications were rare in uremic children, but visceral calcifications were found in a subset at autopsy.

    Conclusions:

    • Vascular calcifications are prevalent in adult dialysis patients but may not progress rapidly.
    • The development of vascular calcifications in dialysis patients is complex and multifactorial.
    • Visceral calcifications may occur in uremic children even without apparent vascular calcifications on X-rays.
