Summary

Clarithromycin (CLARI) shows minimal effectiveness against Mycobacterium tuberculosis. High minimum inhibitory concentrations (MICs) in vitro and lack of therapeutic effect in mouse models indicate CLARI is inactive as a single drug therapy.

Background:

Purpose of the Study:

  • To evaluate the in vitro and in vivo efficacy of clarithromycin (CLARI) against Mycobacterium tuberculosis.
  • To determine the minimum inhibitory concentrations (MICs) of CLARI for M. tuberculosis strains.
  • To assess the therapeutic effect of CLARI in a murine model of tuberculosis.

Main Methods:

  • MICs of CLARI were determined for 12 M. tuberculosis strains using Mueller-Hinton agar supplemented with oleic acid, albumin, dextrose, and catalase.

  • In vivo efficacy was assessed in two mouse experiments involving prolonged treatment with CLARI.
  • Bactericidal and bacteriostatic effects were evaluated in treated mice.
  • Main Results:

    • The MICs of CLARI for 50% and 90% of M. tuberculosis strains were 64 and >128 µg/ml, respectively.
    • These MICs exceed achievable peak serum and lung tissue concentrations of CLARI in humans.
    • Four to six weeks of CLARI treatment in mice showed no bactericidal or bacteriostatic activity against M. tuberculosis.

    Conclusions:

    • Clarithromycin (CLARI) demonstrates poor in vitro activity against Mycobacterium tuberculosis.
    • CLARI concentrations achieved in vivo are unlikely to be effective against M. tuberculosis.
    • Clarithromycin (CLARI) is considered inactive as a single agent for tuberculosis treatment.
