Summary

Cardiac sarcoidosis significantly increases sudden death risk from arrhythmias. This case highlights clinical, imaging, and treatment response evidence strongly suggesting myocardial sarcoidosis, even without direct heart biopsy confirmation.

Area of Science:

  • Cardiology
  • Immunology
  • Pathology

Background:

  • Cardiac sarcoidosis poses a significant risk of sudden cardiac death.
  • Ventricular dysrhythmias and conduction disturbances are common complications.

Observation:

  • A patient presented with systemic sarcoidosis confirmed radiographically and histologically.
  • While direct cardiac biopsy confirmation was absent, the patient exhibited specific clinical manifestations.

Findings:

  • The patient showed radionuclide imaging abnormalities and significant rhythm disturbances.
  • A notable positive response to steroid therapy further supported the diagnosis.

Implications:

  • These factors collectively provide strong evidence for myocardial involvement in sarcoidosis.
