Summary

Granulomas in liver biopsies are common in primary biliary cirrhosis. However, this study found similar granulomas in primary sclerosing cholangitis, suggesting they are not exclusive to primary biliary cirrhosis.

Purpose of the Study:

  • To investigate the prevalence and characteristics of granulomas in patients with primary sclerosing cholangitis (PSC).
  • To determine if granulomas are pathognomonic for PBC.

Main Methods:

  • Retrospective analysis of liver biopsy specimens from patients with PSC and PBC.
  • Histopathological examination of granuloma morphology, distribution, and association with bile leakage.
  • Exclusion of infectious and other granulomatous etiologies.

Main Results:

  • Granulomas were found in 7% of PSC native livers, similar to 7% in PBC controls.
  • In PSC, granulomas were often a response to extravasated bile, not duct destruction.
  • Granulomas in PSC were noncaseating, non-necrotizing, and found in various liver locations.

Conclusions:

  • Granulomas in chronic ductopenic cholestatic liver disease are not exclusive to PBC.
  • Granulomas can be a feature of PSC, potentially related to bile leakage.
  • The findings challenge the pathognomonic nature of granulomas for PBC.
