Area of Science:

  • Neurology
  • Gastroenterology
  • Critical Care Medicine

Background:

  • Gastrointestinal hemorrhage (GIH) is a known risk in critical illness and head injury.
  • Limited data exists on GIH incidence and outcomes in acute stroke patients.

Purpose of the Study:

  • To determine the frequency of GIH in hospitalized acute stroke patients.
  • To investigate potential causes, clinical course, and outcomes of GIH in this population.

Main Methods:

  • Prospective identification of 613 acute stroke patients (excluding subarachnoid hemorrhage) over 36 months.
  • Review of case notes for 607 patients to identify GIH episodes, risk factors, management, and outcomes.

Main Results:

  • Eighteen patients (3%) experienced GIH, with half being severe.

Related Experiment Videos

  • Hemorrhage patients were older and had more severe strokes.
  • GIH was associated with increased in-hospital mortality (OR 4.6; P=.002).
  • Conclusions:

    • This study estimates the frequency of GIH post-acute stroke.
    • Older patients with severe strokes may face a higher risk of GIH.
    • GIH can adversely impact outcomes in acute stroke patients.
