Summary

Single-photon emission CT (SPECT) is more sensitive than brain CT for detecting hypoperfusion in transient ischemic attack (TIA) patients. However, SPECT without an acetazolamide challenge offers no additional clinical insight into vascular risk factors or etiology.

Background:

  • Transient ischemic attacks (TIAs) can be associated with cerebral infarction and prolonged regional hypoperfusion.
  • Differentiating TIA patients with or without focal hypoperfusion is clinically relevant.

Purpose of the Study:

  • To compare the sensitivity of single-photon emission CT (SPECT) versus brain CT in TIA patients.
  • To evaluate the clinical significance of identifying focal hypoperfusion in TIA patients.

Main Methods:

  • Seventy-six consecutive TIA patients without a stroke history were studied.
  • SPECT and CT findings were analyzed using visual and semiquantitative methods.
  • Data collected included timing of imaging, clinical presentation, TIA history, attack duration, vascular risk factors, and etiology.

  • Overall SPECT sensitivity for detecting hypoperfusion was 36%.
  • SPECT demonstrated significantly higher sensitivity than CT (32% vs. 14%; P=.007).
  • SPECT sensitivity was independent of investigation time, attack duration, TIA history, or clinical presentation; no variables discriminated between patients with or without hypoperfusion.

Conclusions:

  • While SPECT is more sensitive than CT for detecting hypoperfusion in TIA patients, its utility is limited.
  • SPECT performed without an acetazolamide challenge does not provide additional clinically useful information on vascular risk factors or etiology in TIA patients.
