Summary

Dopexamine hydrochloride improved oxygen delivery (DO2) and consumption (VO2) in hyperdynamic septic shock patients. However, the increased DO2 was not fully utilized, suggesting further research is needed to confirm its therapeutic value.

Purpose of the Study:

  • To evaluate the effect of dopexamine hydrochloride on oxygen consumption (VO2) in hyperdynamic septic shock patients.
  • To assess the role of beta 2-adrenoreceptor, dopaminergic 1 (DA1), and dopaminergic 2 (DA2) agonistic properties of dopexamine.

Main Methods:

  • Prospective, single-cohort study in an ICU setting.
  • 29 hyperdynamic septic shock patients received short-term dopexamine hydrochloride infusion (2 microgram/kg/min for 30 min).
  • Hemodynamic parameters and oxygen transport variables were measured at baseline and during/after infusion.

  • Dopexamine significantly increased cardiac index (17%) and oxygen delivery (DO2) (16%).
  • VO2 showed a modest but significant increase (4-9%) based on different measurement methods.

  • Dopexamine hydrochloride significantly increases DO2 and, to a lesser extent, VO2 in hyperdynamic septic shock.
  • The observed increase in VO2 may be partly due to direct calorigenic effects and increased myocardial VO2.
  • Further investigation is required to determine if dopexamine improves oxygenation in jeopardized tissues and its definitive role in septic shock management.
