Summary

Eversion of the anorectum during restorative proctocolectomy (RP) for ulcerative colitis may negatively impact functional outcomes, including reduced resting pressure and impaired sensation. Further prospective trials are needed to confirm these findings.

Background:

  • Restorative proctocolectomy (RP) is a key surgical procedure for ulcerative colitis.
  • Functional outcomes after RP are crucial for patient quality of life.
  • Anorectal function is a significant determinant of success in RP.

Purpose of the Study:

  • To evaluate the impact of anorectal eversion during RP on functional outcomes.
  • To compare functional results between patients undergoing RP with and without anorectal eversion.
  • To assess changes in anorectal pressures, sensation, and continence post-RP.

Main Methods:

  • A cohort of 117 patients undergoing RP with stapled end-to-end ileal pouch-anal anastomosis (EEA) was studied.
  • Patients were divided into two groups: 64 with anorectal eversion and 53 without eversion.

  • Anorectal function was assessed pre- and post-operatively (median 12 months).

    • Main Results:

    • Patients with eversion showed lower maximum resting pressure (69 vs. 80 cmH2O, P < 0.04) and impaired anal canal sensation (P = 0.003-P < 0.001).
    • While the rectoanal inhibitory reflex was preserved in most patients, discrimination between flatus and feces was significantly worse in the eversion group (P < 0.001).
    • The anastomosis level was lower in the eversion group (1 cm vs. 1.5 cm above dentate line).

    Conclusions:

    • Anorectal eversion during RP for ulcerative colitis appears to be associated with poorer functional outcomes compared to standard EEA without eversion.
    • Findings suggest potential negative impacts on anal sphincter pressure, sensory discrimination, and continence.
    • Prospective controlled trials are recommended to validate these observations and guide surgical technique.
