    A novel endoscopic technique, peroral direct cholangioscopy (PDCS), allows direct visualization of the biliary system. This method enables precise examination and accessory use within the common bile duct following endoscopic papillotomy.

    • A new technique, peroral direct cholangioscopy (PDCS), utilizes an 8.8 mm diameter prograde fiberscope.
    • The scope is directly inserted into the biliary system post-endoscopic papillotomy (EPT).

    Findings:

    • PDCS provides entirely accurate visualization of the common bile duct lumen.
    • Excellent image quality is achieved.
    • Accessories can be inserted and controlled directly via the biopsy channel.

    Implications:

    • PDCS offers a direct and precise method for biliary tract evaluation.
