Summary

Pulmonary function tests (PFTs) did not predict lung toxicity in patients receiving mitomycin chemotherapy. While a significant decline in diffusing capacity was observed, it did not correlate with adverse outcomes or severe reactions.

  • Pulmonary toxicity from mitomycin can be severe and life-threatening.

Purpose of the Study:

  • To investigate the utility of pulmonary function tests (PFTs) in monitoring for mitomycin-induced pulmonary toxicity.
  • To assess the correlation between PFT changes and clinical outcomes in lung cancer patients.

Main Methods:

  • Prospective study of 133 patients with metastatic lung cancer.
  • Pulmonary function tests (PFTs) performed at baseline and after three cycles of mitomycin-based chemotherapy.
  • Analysis of clinical course, radiologic studies, and PFT results, including diffusing capacity (DCO).

  • An average decline of 14% in DCO was observed after three cycles (p<0.0001), with no changes in expiratory flows.
  • A significant DCO decline (>20%) occurred in 28% of patients, but this was not associated with a worse prognosis.
  • Severe pulmonary toxicity (5%) included edema and pneumonitis; DCO did not predict these events.

Conclusions:

  • Clinically significant pulmonary toxicity from mitomycin is relatively low (5%).
  • Mitomycin therapy can cause a significant decline in DCO in a substantial proportion of patients.
  • Serial PFTs were not predictive of pulmonary toxicity in this patient cohort.
