Summary

Sudden eye pressure lowering after glaucoma surgery can cause retinal hemorrhages, leading to vision loss. Gradual intraocular pressure reduction is recommended to prevent this complication in glaucoma patients.

Background:

  • Glaucoma filtration surgery aims to reduce intraocular pressure (IOP) in patients with glaucoma.
  • Elevated IOP is a primary risk factor for glaucomatous optic neuropathy and vision loss.

Observation:

  • Two cases of young, healthy men with high preoperative IOP experienced retinal hemorrhages post-glaucoma surgery.
  • Hemorrhages were observed in the macula, including the fovea, with some showing white centers.

Findings:

  • Retinal hemorrhages occurred following ocular decompression in glaucomatous eyes after filtration surgery.
  • Both patients sustained permanent visual acuity decrease after hemorrhage resolution.
