Summary

Three patients experienced prolonged mechanical ventilation due to acute respiratory failure. Investigations revealed underlying motor neuron disease, leading to difficulties in weaning and eventual fatal outcomes.

Background:

Acute respiratory failure necessitates mechanical ventilation.
Weaning failure from artificial respiration presents a clinical challenge.

Observation:

Three elderly male patients required prolonged mechanical ventilation.
Initial causes of respiratory failure were unclear.
Patients could not be weaned from artificial respiration.

Findings:

Diagnosis of motor neuron disease (MND) was established.
Two patients had progressive spinal muscular atrophy (SMA).
The third patient exhibited SMA with thoracic hydromelia.
