    Background:

    • Complete dislocation of the odontoid process anterior to the C1 arch is a rare and severe cervical spine injury.
    • This injury pattern, without associated C1 arch fracture, presents unique management challenges.

    Observation:

    • A patient presented with a complete dislocation of the odontoid process anterior to the anterior arch of the first cervical vertebra (C1).
    • Radiographic evaluation confirmed no fracture of the C1 vertebral arch.

    Findings:

    • The patient underwent successful trans-oral excision of the odontoid process and the anterior arch of C1.
    • Subsequent posterior spinal stabilization from C1 to C3 was achieved using wire and methyl-methacrylate fixation.

    Implications:

    • Trans-oral odontoidectomy is a feasible surgical option for managing complete anterior C1-odontoid dislocation without C1 fracture.
    • Combined anterior and posterior surgical approaches can effectively stabilize the cervical spine following this rare injury.
