Summary

This clinical trial found no significant difference in acetabular osteolysis prevalence between cemented and cementless total hip arthroplasty. However, titanium femoral heads were associated with osteolysis in both fixation methods.

Area of Science:

  • Orthopedic Surgery
  • Biomaterials Science
  • Radiology

Background:

  • Acetabular osteolysis is a concern in total hip arthroplasty.
  • Cementless fixation is often preferred to minimize osteolysis.
  • Titanium components are widely used in hip implants.

Purpose of the Study:

  • To compare the prevalence and pattern of acetabular osteolysis between cemented and cementless total hip arthroplasty.
  • To evaluate the role of titanium femoral heads in osteolysis development.

Main Methods:

  • Prospective, randomized, double-blind clinical trial.
  • Comparison of cemented versus cementless acetabular components.
  • Radiographic assessment of osteolysis at 2-6 year follow-up in 224 patients.

Related Experiment Videos

Main Results:

  • No significant difference in acetabular osteolysis prevalence: 5% cemented vs. 9% cementless.
  • Titanium femoral heads showed a trend towards causing osteolysis in both groups.
  • Osteolysis patterns differed: predominantly zone 1 (cemented) vs. zone 2 (cementless).

Conclusions:

  • Cementless acetabular fixation does not significantly reduce osteolysis prevalence compared to cemented fixation.
  • Titanium femoral heads may contribute to osteolysis regardless of fixation method.
  • Understanding osteolysis patterns is crucial for implant design and surgical technique.
