Summary

Duplex ultrasound is as accurate as arteriography for detecting aorto-iliac disease. This study suggests aorto-iliac reconstruction can proceed without arteriography if duplex scans show total occlusion.

Area of Science:

  • Vascular Surgery
  • Diagnostic Imaging
  • Medical Technology

Background:

  • Duplex imaging demonstrates accuracy comparable to arteriography for identifying significant aorto-iliac stenosis or occlusions.
  • Arteriography has been the traditional gold standard for evaluating aorto-iliac disease.

Purpose of the Study:

  • To assess the role of duplex scanning in guiding the selection of procedures for primary aorto-iliac reconstructions.
  • To determine if arteriography is necessary when duplex imaging identifies aorto-iliac occlusive disease.

Main Methods:

  • A retrospective review of 54 patients undergoing primary aorto-iliac reconstruction over 18 months.
  • Analysis of duplex scan and arteriogram reports to identify deviations from planned procedures.
  • Assessment of duplex scan's influence on procedural planning and execution.

Related Experiment Videos

Main Results:

  • 13 patients had both duplex and arteriography, 30 had arteriography only, and 11 had duplex only.
  • Total occlusion of the aorta or iliac artery was present in 10 (group I), 15 (group II), and 9 (group III) patients.
  • No patients who underwent duplex scanning alone required deviation from the preoperatively planned reconstruction.

Conclusions:

  • Duplex scanning is a reliable tool for diagnosing hemodynamically significant aorto-iliac disease.
  • Arteriography may be safely omitted in cases of total aorto-iliac occlusion identified by duplex imaging.
  • This approach can streamline the diagnostic and planning process for aorto-iliac reconstruction.
