Summary

A delayed suture granuloma occurred 7 years after medial canthal tendon surgery. Surgical removal of the granuloma and suture resolved symptoms without needing further tendon tightening.

Background:

  • Medial canthal tendon (MCT) tightening is a common surgical procedure to address eyelid laxity.
  • Complications, though rare, can arise years after initial surgery.

Observation:

  • An 81-year-old female presented with a suture granuloma 7 years post-MCT tightening.
  • A suture end was found to have migrated through the inferior punctum.

Findings:

  • Excision of the granuloma and the offending suture provided complete symptom relief.
  • No recurrence of purulent discharge, foreign-body sensation, or epiphora was noted after simple suture removal.

Implications:

  • This case highlights the potential for late-onset complications from retained sutures after MCT surgery.
  • Conservative management (suture excision) may be sufficient for resolving such granulomas, avoiding more extensive revision surgery.
