Summary

A normal rectal biopsy does not always rule out ulcerative colitis (UC). In nearly half of established UC cases, rectal biopsies may appear normal, highlighting the need for clinical context in diagnosis.

Purpose of the Study:

  • To determine if a normal rectal biopsy can occur in patients with a confirmed diagnosis of ulcerative colitis.
  • To evaluate the diagnostic accuracy of rectal biopsies in established UC cases.

Main Methods:

  • A blinded study analyzed rectal biopsy specimens from 24 asymptomatic patients with established UC and 10 controls.
  • Biopsies were categorized by three pathologists into: normal, borderline, minimal UC changes, or unequivocal UC.
  • Histological and clinical data were reviewed for prognostic markers.

  • Pathologists must consider the minimal spectrum of UC disease and integrate biopsy findings with clinical and radiological data.
