Background:

Primary HIV infection, occurring during seroconversion, frequently causes symptomatic illness in over 50% of individuals.
The typical presentation resembles mononucleosis or influenza, but HIV is a known clinical mimic.
Recognition of diverse primary HIV infection symptoms is crucial for timely diagnosis and management.

Observation:

Six cases of symptomatic primary HIV infection are presented.
One case exhibited a classic presentation.
Five cases demonstrated atypical symptoms including colitis, rhabdomyolysis, epiglottitis, cold agglutination hemolytic anemia, and meningitis.

Findings:

Primary HIV infection can manifest with a wide spectrum of clinical symptoms beyond the typical flu-like illness.
Uncommon presentations such as colitis, rhabdomyolysis, and meningitis can be the predominant features.
The diagnostic challenge lies in HIV's ability to imitate various acute conditions.

Implications:

Increased awareness of atypical presentations is vital for clinicians diagnosing primary HIV infection.
Prompt diagnosis facilitates early initiation of antiretroviral therapy, improving patient outcomes.
Understanding the varied clinical spectrum aids in differentiating HIV from other acute febrile illnesses.
