    Background:

    Observation:

    • Three cases of acute carbamazepine intoxication were assessed using neurotological and neurological examinations.
    • Observed symptoms included equilibrium, gait, and speech disturbances, drowsiness, gaze nystagmus, depressed optokinetic nystagmus, and impaired smooth pursuit eye movements.
    • These neurological signs mimicked those of a space-occupying lesion.

    Findings:

    • Neurological symptoms resolved completely after discontinuation of carbamazepine.
    • The findings suggest carbamazepine impacts both brain stem and cerebellar structures.
    • Equilibrium disorders in this context arise not solely from cerebellar lesions but also from brain stem involvement.

    Implications:

    • Carbamazepine-induced neurological deficits can affect both the cerebellum and brain stem.
    • This expands the understanding of the neurotoxic effects of carbamazepine.
    • Clinicians should consider brain stem involvement when evaluating patients with carbamazepine intoxication and associated neurological symptoms.
