    Summary

    Primary amyloidosis rarely presents as a tumorlike mass in the airway. Tracheal sleeve resection is a viable, economical option for localized lesions, but carries risks of stenosis.

    Background:

    • Primary amyloidosis is a rare systemic disease.
    • Tumorlike formations in the tracheobronchial tree are exceptionally uncommon manifestations.
    • Surgical resection is a treatment option, but tracheal or bronchial sleeve resection has not been previously reported.

    Observation:

    • A case of tumorlike amyloidosis in the trachea was successfully treated with tracheal sleeve resection.
    • The lesion was well-circumscribed to a short segment of the trachea.
    • Amyloid deposits were present in the mucosa above and below the main lesion.

    Findings:

    • Tracheal sleeve resection can be a satisfactory and economical surgical solution for localized, tumorlike amyloidosis.
    • The primary risk associated with sleeve resection is anastomotic stenosis due to undermined mucosa.
    • Piecemeal endoscopic removal may offer a better prognosis for extensive or less-circumscribed amyloidosis.

    Implications:

    • This case expands the surgical options for managing rare airway amyloidosis.
    • Careful assessment of mucosal involvement is crucial when considering sleeve resection.
    • Endoscopic removal should be prioritized for extensive disease to minimize risks and improve outcomes.
