A 26-year-old man experienced sudden vision loss due to HLA-B27-associated uveitis. Diffuse vitritis, though uncommon, was the primary symptom, responding well to steroid treatment.

The patient had a history of pauciarticular juvenile rheumatoid arthritis and ankylosing spondylitis, associated with the HLA-B27 haplotype.
