One-stage anterior urethroplasty

Summary

This study presents a novel urethroplasty technique using penile skin to reconstruct the urethra in patients with strictures or hypospadias. The one-stage procedure achieved successful outcomes with minimal complications.

Background:

Anterior urethral strictures and hypospadias often require complex surgical correction.
Previous repair attempts or severe congenital defects can lead to insufficient penile or preputial skin for reconstruction.

Purpose of the Study:

To evaluate a one-stage urethroplasty technique using penile skin to replace deficient urethral segments.
To address long, tight urethral strictures, failed hypospadias repairs, and severe hypospadias with circumcision.

Main Methods:

A pedicled circumferential strip of distal penile skin was used to create the neourethral floor in 29 patients.
The neourethral roof was formed by epithelial regeneration over Buck's fascia.
Indications included anterior urethral stricture (11), failed hypospadias repair (6), and severe hypospadias with circumcision (12).

Main Results:

Successful neourethra construction with adequate caliber and length in all patients.
Minimal postoperative complications were observed, including 2 urethrocutaneous fistulas, 1 meatal stenosis, 1 persistent chordee, and 1 distal penile skin patch slough.
Mean follow-up duration was 19 months.

Conclusions:

This urethroplasty technique is effective for correcting anterior urethral strictures.
The method is also suitable for hypospadias repair when penile or preputial skin is insufficient.
The one-stage procedure offers a viable solution for complex urethral reconstruction.
