    Background:

    • Thyroid hormone levels are crucial for diagnosing and managing thyroid disorders.
    • Radioimmunoassay (RIA) is a common method for measuring thyroid hormone concentrations.

    Observation:

    • A thyrotoxic patient on propylthiouracil showed significantly elevated triiodothyronine levels by RIA, despite mild symptoms.
    • A euthyroid patient, recovering from subacute thyroiditis, also had elevated serum triiodothyronine.
    • Gel electrophoresis revealed abnormal binding of triiodothyronine and thyroxine to gamma globulins in both patients.

    Findings:

    • Patient 1 exhibited binding of both triiodothyronine and thyroxine to gamma globulins.
    • Patient 2 showed binding of triiodothyronine to gamma globulins.
    • These findings suggest that abnormal globulin binding can lead to falsely elevated thyroid hormone measurements.

    Implications:

    • Abnormal globulin binding of thyroid hormones can cause significant errors in RIA-based diagnostics.
    • This phenomenon necessitates careful interpretation of thyroid hormone levels, especially in patients with suspected binding abnormalities.
    • Further research is needed to understand the prevalence and clinical significance of such binding disorders.
