    Mitral leaflet prolapse may cause visual disturbances due to embolism, though evidence is not conclusive. Cardiovascular assessment, including echocardiography, is recommended for patients with acute cerebral or ocular ischemia.

    Background:

    • Mitral leaflet prolapse (MLP) is a common valvular condition.
    • Visual disturbances can indicate serious neurological or vascular events.
    • Embolism in the ophthalmic or posterior cerebral circulation can lead to vision loss.

    Observation:

    • Four young women and six older men with MLP experienced visual disturbances.
    • Symptoms were consistent with embolism in the ophthalmic or posterior cerebral arteries.
    • Cardiac arrhythmias were frequently observed in these patients.

    Findings:

    • The association between MLP and embolism causing visual disturbances is suggestive but not definitive.
    • Cardiac arrhythmias are rarely linked to focal cerebral ischemia.
    • Further research is required to establish a conclusive link.

    Implications:

    • Patients presenting with acute cerebral or ocular ischemia require thorough cardiovascular evaluation.
    • Routine echocardiography should be part of the cardiovascular assessment for these patients.
    • Understanding the link between MLP and embolic events can improve diagnostic and treatment strategies.
