Summary

This study presents a rare case of isolated thyroid-stimulating hormone (TSH) deficiency in a 60-year-old female. Diagnosis involved assessing thyroid hormone levels, TSH response to TRH, and other pituitary hormones, confirming the deficiency.

Area of Science:

  • Endocrinology
  • Reproductive Biology
  • Molecular Medicine

Background:

  • Isolated TSH deficiency is a rare endocrine disorder characterized by insufficient thyroid-stimulating hormone (TSH) production.
  • Diagnosis relies on clinical signs of hypothyroidism, impaired TSH response to thyrotropin-releasing hormone (TRH), and normal function of other pituitary hormones.

Observation:

  • A 60-year-old female presented with symptoms of thyroid hormone deficiency.
  • Laboratory results showed low free thyroxine and total triiodothyronine with a suppressed TSH level.
  • Repeated TRH tests confirmed an inadequate TSH response, and CT imaging revealed a partially empty sella.

Findings:

  • The patient was diagnosed with isolated TSH deficiency.
  • Other pituitary hormones were found to be intact.

Related Experiment Videos

  • Pituitary antibodies were not detected via immunofluorescence.
  • Implications:

    • This case highlights the diagnostic criteria for isolated TSH deficiency.
    • Treatment with thyroxine effectively alleviated symptoms and further suppressed TSH.
    • Understanding rare pituitary disorders is crucial for accurate diagnosis and management.
