Tentorial dural flap for transtentorial surgery

A specific incision is made at the tentorial free edge, extending towards the superior petrosal sinus.
A triangular flap of tentorial dura is created and everted over the superior petrosal sinus.

The technique provides a simple and safe method for creating a wide surgical window.
It effectively protects the fourth and fifth cranial nerves.
Bleeding from the superior petrosal sinus and petrosal vein is avoided.

Conclusions:

The described tentorial dural flap is a valuable addition to neurosurgical approaches to the anterior posterior fossa.
It enhances surgical exposure, improves safety, and simplifies the procedure.
