Background:

  • Leprosy treatment often involves multidrug therapy, including clofazimine.
  • Minocycline is sometimes used as an alternative or adjunct therapy.

Observation:

  • A patient with borderline lepromatous leprosy developed red leprosy plaques after 9 months of dapsone, rifampicin, and clofazimine.
  • After switching from clofazimine to minocycline, the patient developed blue-black pigmentation in lesions.

Findings:

  • Histopathology confirmed minocycline-induced hyperpigmentation in the leprosy lesions.
  • This is the first documented case of minocycline-induced pigmentation in a leprosy patient.

Implications:

  • Clinicians should be aware of minocycline-induced hyperpigmentation as a potential side effect in leprosy treatment.
