Summary

This study found that 41% of Skilled Nursing Facility patients acquired infections, with urinary tract infections being most common. Antibiotic resistance, particularly methicillin-resistant Staphylococcus aureus, is a significant concern in this setting.

Area of Science:

  • Infectious Diseases
  • Antimicrobial Resistance
  • Healthcare Epidemiology

Background:

  • Nosocomial infections and antibiotic use are significant challenges in Skilled Nursing Facilities (SNFs).
  • Understanding infection types, antibiotic utilization, and resistance patterns is crucial for effective patient care and infection control.

Purpose of the Study:

  • To survey infection types, antibiotic utilization, and bacterial pathogen resistance patterns in a community SNF.
  • To establish baseline data for antimicrobial stewardship and infection prevention initiatives.

Main Methods:

  • A 20-month prospective observational surveillance program was conducted in a 149-bed community SNF.
  • Data were collected from 585 admitted patients via medical charts, laboratory reports, and nurses' reports.

Related Experiment Videos

Main Results:

  • 41% of patients developed at least one presumptive nosocomial infection; 54% received antibiotic treatment.
  • Urinary tract (38%) and respiratory tract (28%) infections were most common. E. coli was the most frequent pathogen.
  • Quinolones were the most used antibiotic group (22%). Methicillin-resistant Staphylococcus aureus (MRSA) was found in 39% of S. aureus isolates, with 94% also resistant to ciprofloxacin, often associated with catheter-associated UTIs.

Conclusions:

  • High rates of nosocomial infections and significant antibiotic resistance, particularly MRSA, were observed in the SNF.
  • Targeted interventions focusing on urinary tract infections and specific resistant pathogens like MRSA are warranted.
