    Summary

    In elderly individuals, a single second heart sound is not typical. However, reverse splitting of the second heart sound may indicate severe latent heart disease.

    Background:

    • The characteristics of the second heart sound (S2) in aging populations require further investigation.
    • Understanding S2 variations in the elderly is crucial for identifying potential cardiac abnormalities.

    Purpose of the Study:

    • To investigate the phonocardiography and carotid tracings of the second heart sound in elderly subjects.
    • To determine if a single second sound is a common finding in older adults.
    • To assess the significance of abnormal S2 splitting in the elderly.

    Main Methods:

    • Phonocardiography and carotid tracings were utilized.
    • 103 elderly subjects (aged 60-99) without overt heart disease were studied.
    • Findings were compared to 60 younger, healthy individuals.

    Main Results:

    • A normal S2 splitting pattern was observed in 55.3% of elderly subjects.
    • A single second sound was present in 41.7% of elderly subjects.
    • The prevalence of normal splitting and single sounds did not significantly differ between elderly and younger groups.

    • Reverse splitting of the second heart sound, though rare (3 patients), is indicative of severe latent heart disease in the elderly.
