Summary

We describe a rare case of actinomycosis with unique computed tomographic (CT) findings, including a retroperitoneal mass causing hydronephrosis. CT imaging also proved useful in tracking the patient's response to antibiotic therapy.

Area of Science:

  • Medical Imaging
  • Infectious Diseases
  • Surgical Pathology

Background:

  • Actinomycosis is a rare bacterial infection often presenting with chronic, granulomatous inflammation.
  • Retroperitoneal masses can have diverse etiologies, posing diagnostic challenges.

Observation:

  • A pathologically confirmed case of actinomycosis presented with unusual computed tomographic (CT) features.
  • The CT scan revealed a soft-tissue retroperitoneal mass encasing the ureter, leading to hydronephrosis.

Findings:

  • The retroperitoneal mass extended into the intraperitoneal space in a distinct stellate pattern.
  • Computed tomography (CT) demonstrated its utility in monitoring therapeutic response to antibiotics.

Implications:

  • This case highlights the importance of considering actinomycosis in the differential diagnosis of retroperitoneal masses with atypical imaging findings.
  • CT imaging plays a crucial role in both diagnosis and treatment monitoring of complex actinomycosis cases.
