Summary

The posterior interhemispheric approach significantly reduces the risk of olfactory dysfunction after surgery for anterior communicating artery aneurysms compared to the subfrontal/interhemispheric approach. This improvement is due to less exposure of olfactory nerves during the posterior approach.
