Background:

Primary hyperparathyroidism (PHP) involves excessive parathormone secretion, disrupting calcium-phosphate balance and bone metabolism.
Historically considered rare, PHP is now frequently diagnosed in subtle or asymptomatic phases, increasing clinical interest.

Observation:

The authors present case studies highlighting diagnostic challenges in identifying PHP.
Difficulties in localizing the source of excessive parathormone secretion are discussed.

Findings:

PHP diagnosis is becoming more common, even with minimal symptoms.
Effective differential diagnosis and localization are key to managing PHP.

Implications:
