Summary

Interleukin-2 (IL2) immunotherapy can cause new neurologic problems, including confusion and seizures. Most patients recovered after stopping IL2 treatment, with brain lesions resolving on scans.

Background:

  • Interleukin-2 (IL2)-based immunotherapy is used for malignancy and HIV infection.
  • Neurologic complications can arise during IL2 treatment.

Observation:

  • Eight patients receiving IL2 developed new focal neurologic abnormalities.
  • Symptoms progressed from confusion and delirium to coma, ataxia, hemiparesis, seizures, and cortical syndromes.
  • Radiologic studies revealed multiple white and gray matter lesions, particularly in the occipital poles, centrum semiovale, and cerebellum.

Findings:

  • Cessation of IL2 treatment led to neurologic improvement in seven patients.
  • Resolution of brain lesions on imaging studies paralleled clinical recovery.
  • One patient experienced only minimal improvement.
