Early post-operative mortality following primary total hip replacement

Summary

This study assessed the risk of death after total hip replacement (THR). The excess risk of death in the first three months post-surgery was found to be approximately 1.6 times higher than expected.

Background:

Total hip replacement (THR) is a common surgical procedure for hip osteoarthritis.
Assessing the perioperative mortality risk is crucial for patient selection and informed consent.

Purpose of the Study:

To determine the excess risk of death following primary total hip replacement (THR).
To compare mortality rates between patients who underwent THR and those on the waiting list.

Main Methods:

A cohort study design was employed, including patients undergoing THR and those on the waiting list over an 18-month period.
Mortality data were obtained from the Registrar General for Scotland.
Crude mortality rate, standardized mortality ratio (SMR), relative mortality ratio, and comparative mortality ratio were calculated.

The crude mortality rate for the operated-upon group was 1.8%.
The standardized mortality ratio (SMR) for the operated-upon group was 45.5.
The excess risk of death within the first 3 post-operative months was approximately 1.6 (comparative mortality ratio).

Conclusions:

Patients selected for THR are generally healthier than the average population.
The excess mortality risk associated with THR in the early post-operative period (first 3 months) is relatively low, around 1.6.
