Plasma exchange therapy for solar urticaria

Summary

Plasma exchange therapy (PExT) was investigated for severe idiopathic solar urticaria. While one patient showed initial improvement, lasting benefits were not demonstrated for PExT in treating this condition.

Background:

Idiopathic solar urticaria (ISU) is a rare photodermatosis causing hives upon sun exposure.
Standard treatments like antihistamines and phototherapy offer limited efficacy for severe ISU cases.

Observation:

Two patients with severe ISU, refractory to conventional therapies including antihistamines and phototherapy (PUVA, NB-UVB), underwent plasma exchange therapy (PExT).
Therapeutic response was evaluated using detailed phototesting and serum assays.

Findings:

One patient experienced a temporary improvement in solar urticaria symptoms following PExT.
This positive response was short-lived, with relapse occurring within two weeks.
Overall, PExT did not provide a sustained therapeutic benefit for the studied ISU patients.

Implications:

Plasma exchange therapy is unlikely to be an effective long-term treatment for severe idiopathic solar urticaria.
Further research is needed to explore alternative or adjunctive therapies for refractory photodermatoses.
Understanding the underlying mechanisms of ISU may guide the development of more targeted treatments.
