Summary

This study introduces an oblique Fadenoperation technique for strabismus surgery. The modified procedure effectively reduced vertical deviation and excyclotropia in patients with incomitant vertical deviations.

Context:

Cüppers' Fadenoperation principle involves reducing lever arm and lengthening arc of contact.
A novel surgical approach modifies the Fadenoperation by creating an oblique muscle adherence to the sclera.

Purpose:

To describe a modified Fadenoperation surgical procedure adhering to Cüppers' principle.
To evaluate the efficacy of an oblique retroequatorial fixation technique in strabismus correction.

Summary:

Five patients underwent oblique retroequatorial fixation (4 inferior recti, 1 superior rectus) for various palsies and orbital fractures.
The oblique Fadenoperation in four patients successfully reduced vertical deviation, comparable to classic posterior fixation.
A notable secondary benefit included a slight reduction in excyclotropia in downgaze.

Impact:

The oblique Fadenoperation is a viable alternative for treating incomitant vertical deviations.
This technique offers potential benefits for patients with specific strabismus types, including excyclotropia in downgaze.
The procedure demonstrates anatomical feasibility and clinical effectiveness in a small patient cohort.
