Severe hypothyroidism can mimic polymyositis and cause kidney issues. Prompt thyroid replacement therapy resolved all patient symptoms, highlighting the importance of thyroid-stimulating hormone (TSH) testing in renal impairment.

Background:

Observation:

  • A 51-year-old male presented with proximal muscle weakness, stiffness, pain, rhinorrhea, and nasal stenosis.
  • Initial investigations revealed elevated S-creatine-kinase, reduced creatinine clearance, and proteinuria.
  • Skin, muscle, and kidney biopsies were unremarkable, ruling out polymyositis and secondary glomerulopathy.

Findings:

  • All patient symptoms resolved within three months of initiating thyroid replacement therapy.
