Fulminant Clostridium difficile enteritis and proctitis can occur after prostatectomy, even in patients with prior ileorectal anastomosis. This rare complication highlights the potential for small intestine involvement in C. difficile infections.

Background:

  • Clostridium difficile infection (CDI) commonly affects the colon, leading to pseudomembranous colitis.
  • Small bowel involvement in CDI is considered rare, particularly in adults.

Observation:

  • A 71-year-old male with a history of ileorectal anastomosis developed severe, fatal enteritis and proctitis post-prostatectomy.
  • The patient's condition was attributed to Clostridium difficile infection.

Findings:

  • This case, along with three other adult reports, demonstrates that the small intestine is susceptible to C. difficile.
  • No prior reports documented C. difficile enteritis following colectomy in this manner.
