    Area of Science:

    • Urology
    • Plastic Surgery
    • Pediatric Surgery

    Background:

    • Prostatic fistulas, particularly in cases of exstrophy and epispadias, present significant closure challenges.
    • Secondary breakdown after simple fistula closure is a common complication, leading to poor outcomes.

    Purpose of the Study:

    • To describe and evaluate a novel surgical technique for the closure of difficult prostatic fistulas.
    • To assess the efficacy and long-term outcomes of this technique in patients with complex genitourinary anomalies.

    Main Methods:

    • A rotated, twisted full-thickness skin flap technique was employed for fistula closure.
    • The flap's epidermal tip formed the fistula roof, while the pedicle was buried after epidermal removal to minimize tension.

    Main Results:

    • Excellent long-term results were observed in two patients treated with this technique.
    • The tension-free closure significantly reduced the likelihood of breakdown.

    Conclusions:

    • This rotated skin flap technique offers a consistent and effective method for closing difficult prostatic fistulas.
    • The technique is adaptable to other challenging closure scenarios prone to secondary breakdown.
