Summary

A blood group discrepancy between red blood cells and serum was resolved. The individual possesses a rare cis-AB blood group with a weak B antigen variant, known as Bel.

Background:

Blood group discrepancies between red cell and serum testing can indicate rare antigen variants.
Accurate blood typing is crucial for safe blood transfusions and transplantation.

Observation:

A patient presented with conflicting red cell (Group A) and serum (Group AB) typing results.
Further serological testing, including adsorption-elution and saliva tests, was performed to investigate the discrepancy.

Findings:

The comprehensive analysis confirmed the blood group as AB.
A weak B antigen variant, identified as Bel, was detected.
Genetic analysis of parental blood groups (mother AB, father O) supported the findings.
